M — Massive Haemorrhage

  1. Look for injuries causing the most amount of blood loss. More red = more injured.
  2. When multiple injuries are present on a limb, or the limb is broken, apply a tourniquet.
    • Make sure every major injury is treated before moving on to the next step.

A — Airways

  1. Check if the patient is breathing.
  2. Perform the “Check Airway” action. If any collapse is present, perform “Head-Tilt Chin-Lift”
  3. If any obstruction is noted, perform head turning.
  4. If note “Medical suction required” use Suction bag or inform medic of blocked airway (URGENT).

R — Respiration

  1. Check that the patient is respiring (lung function).
  2. Inspect the chest for rise and fall:
    • Normal — continue to Circulation.
    • Uneven — apply chest seal; check for normal rise/fall.
      • If still uneven, utilise NCD kit.
    • Uneven with heavy bruising — patient is immediate. Apply chest seal and inform medical staff of “Hemothorax” (bleeding in pleural space).

C — Circulation

  1. Confirm the presence of a pulse.
  2. Check via “Check Pulse” action.
    • Check the pulse on a non-tourniqueted limb.
  3. If a pulse is observed, put the patient into a recovery position.
  4. If no pulse is observed, perform CPR while a helper bandages all reopening wounds and keeps airways clear.
  5. After a recovery position is established, check for new bleeding every 1–2 minutes.

H — Hypothermia

  1. Check patient temperature - We are looking for “Warm” or “Mildly Cool”
  2. If cold, apply handwarmers/heating pads & administer fluids.

Aftercare

After MARCH is done, put the patient in the recovery position and repeat MARCH checks frequently until a medic arrives.
